Root Cause Analysis (RCA) Process

Modified on Thu, 7 Jul, 2022 at 12:49 PM

As part of the NAVBLUE Quality Management System (QMS), a Root Cause Analysis (RCA) is typically conducted when any nonconformity is identified and reported either internally or by our customers.

The RCA is the process of discovering the root causes of problems in order to identify the appropriate solution(s).

An RCA will typically be conducted by a multifunctional team (MFT) who can review the end to end process and fully apply the 5 why analysis to get to the true root cause.  Through the RCA process, containment, corrective and/or preventative actions may be identified.

Containment action: Immediate actions taken to contain and limit the impact of any event or non conformity

Corrective actions:  Measures that are identified and implemented to restore service and enable normal operations to resume

Preventative actions: Changes that are identified to prevent any future reoccurrence

NAVBLUE will provide details of the RCA upon request and where mutually agreed for safety related or urgent criticality issues
